Please note the Nebraska Forest Service is not a regulatory agency. We work cooperatively with the Nebraska Department of Agriculture to document and/or assess reported damages to trees. This form helps us capture the frequency of herbicide-related damage across Nebraska. If you wish to file a formal complaint, please review NDA's procedure.
